Research is indispensable for resolving public health challenges – whether it be tackling diseases of poverty, responding to rise of chronic diseases,  or ensuring that mothers have access to safe delivery practices.

Likewise, shared vulnerability to global threats, such as severe acute respiratory syndrome, Ebola virus disease, Zika virus and avian influenza has mobilized global research efforts in support of enhancing capacity for preparedness and response. Research is strengthening surveillance, rapid diagnostics and development of vaccines and medicines.

Public-private partnerships and other innovative mechanisms for research are concentrating on neglected diseases in order to stimulate the development of vaccines, drugs and diagnostics where market forces alone are insufficient.

Research for health spans 5 generic areas of activity:

  • measuring the magnitude and distribution of the health problem;
  • understanding the diverse causes or the determinants of the problem, whether they are due to biological, behavioural, social or environmental factors;
  • developing solutions or interventions that will help to prevent or mitigate the problem;
  • implementing or delivering solutions through policies and programmes; and
  • evaluating the impact of these solutions on the level and distribution of the problem.

High-quality research is essential to fulfilling WHO’s mandate for the attainment by all peoples of the highest possible level of health. One of the Organization’s core functions is to set international norms, standards and guidelines, including setting international standards for research.

Under the “WHO strategy on research for health”, the Organization works to identify research priorities, and promote and conduct research with the following 4 goals:

  • Capacity - build capacity to strengthen health research systems within Member States.
  • Priorities - support the setting of research priorities that meet health needs particularly in low- and middle-income countries.
  • Standards - develop an enabling environment for research through the creation of norms and standards for good research practice.
  • Translation - ensure quality evidence is turned into affordable health technologies and evidence-informed policy.

Understanding Clinical Trials

Clinical research: what is it.

Your doctor may have said that you are eligible for a clinical trial, or you may have seen an ad for a clinical research study. What is clinical research, and is it right for you?

Clinical research is the comprehensive study of the safety and effectiveness of the most promising advances in patient care. Clinical research is different than laboratory research. It involves people who volunteer to help us better understand medicine and health. Lab research generally does not involve people — although it helps us learn which new ideas may help people.

Every drug, device, tool, diagnostic test, technique and technology used in medicine today was once tested in volunteers who took part in clinical research studies.

At Johns Hopkins Medicine, we believe that clinical research is key to improve care for people in our community and around the world. Once you understand more about clinical research, you may appreciate why it’s important to participate — for yourself and the community.

What Are the Types of Clinical Research?

There are two main kinds of clinical research:

Observational Studies

Observational studies are studies that aim to identify and analyze patterns in medical data or in biological samples, such as tissue or blood provided by study participants.

Clinical trials, which are also called interventional studies, test the safety and effectiveness of medical interventions — such as medications, procedures and tools — in living people.

Clinical research studies need people of every age, health status, race, gender, ethnicity and cultural background to participate. This will increase the chances that scientists and clinicians will develop treatments and procedures that are likely to be safe and work well in all people. Potential volunteers are carefully screened to ensure that they meet all of the requirements for any study before they begin. Most of the reasons people are not included in studies is because of concerns about safety.

Both healthy people and those with diagnosed medical conditions can take part in clinical research. Participation is always completely voluntary, and participants can leave a study at any time for any reason.

Types of Research Studies

Within the two main kinds of clinical research, there are many types of studies. They vary based on the study goals, participants and other factors.

Biospecimen studies

Healthy volunteer studies.

Clinical trials study the safety and effectiveness of interventions and procedures on people’s health. Interventions may include medications, radiation, foods or behaviors, such as exercise. Usually, the treatments in clinical trials are studied in a laboratory and sometimes in animals before they are studied in humans. The goal of clinical trials is to find new and better ways of preventing, diagnosing and treating disease. They are used to test:

 Goals of Clinical Trials

Because every clinical trial is designed to answer one or more medical questions, different trials have different goals. Those goals include:

Treatment trials

Prevention trials, screening trials, phases of a clinical trial.

In general, a new drug needs to go through a series of four types of clinical trials. This helps researchers show that the medication is safe and effective. As a study moves through each phase, researchers learn more about a medication, including its risks and benefits.

Is the medication safe and what is the right dose?   Phase one trials involve small numbers of participants, often normal volunteers.

Does the new medication work and what are the side effects?   Phase two trials test the treatment or procedure on a larger number of participants. These participants usually have the condition or disease that the treatment is intended to remedy.

Is the new medication more effective than existing treatments?  Phase three trials have even more people enrolled. Some may get a placebo (a substance that has no medical effect) or an already approved treatment, so that the new medication can be compared to that treatment.

Is the new medication effective and safe over the long term?   Phase four happens after the treatment or procedure has been approved. Information about patients who are receiving the treatment is gathered and studied to see if any new information is seen when given to a large number of patients.

Is It Safe to Participate in Clinical Research?

There are several steps in place to protect volunteers who take part in clinical research studies. Clinical Research is regulated by the federal government. In addition, the institutional review board (IRB) and Human Subjects Research Protection Program at each study location have many safeguards built in to each study to protect the safety and privacy of participants.

Clinical researchers are required by law to follow the safety rules outlined by each study's protocol. A protocol is a detailed plan of what researchers will do in during the study.

In the U.S., every study site's IRB — which is made up of both medical experts and members of the general public — must approve all clinical research. IRB members also review plans for all clinical studies. And, they make sure that research participants are protected from as much risk as possible.

Earning Your Trust

This was not always the case. Many people of color are wary of joining clinical research because of previous poor treatment of underrepresented minorities throughout the U.S. This includes medical research performed on enslaved people without their consent, or not giving treatment to Black men who participated in the Tuskegee Study of Untreated Syphilis in the Negro Male. Since the 1970s, numerous regulations have been in place to protect the rights of study participants.

Many clinical research studies are also supervised by a data and safety monitoring committee. This is a group made up of experts in the area being studied. These biomedical professionals regularly monitor clinical studies as they progress. If they discover or suspect any problems with a study, they immediately stop the trial. In addition, Johns Hopkins Medicine’s Research Participant Advocacy Group focuses on improving the experience of people who participate in clinical research.

The choice of study type is an important aspect of the design of medical studies. The study design and consequent study type are major determinants of a study’s scientific quality and clinical value.

This article describes the structured classification of studies into two types, primary and secondary, as well as a further subclassification of studies of primary type. This is done on the basis of a selective literature search concerning study types in medical research, in addition to the authors’ own experience.

Three main areas of medical research can be distinguished by study type: basic (experimental), clinical, and epidemiological research. Furthermore, clinical and epidemiological studies can be further subclassified as either interventional or noninterventional.

Conclusions

The study type that can best answer the particular research question at hand must be determined not only on a purely scientific basis, but also in view of the available financial resources, staffing, and practical feasibility (organization, medical prerequisites, number of patients, etc.).

The quality, reliability and possibility of publishing a study are decisively influenced by the selection of a proper study design. The study type is a component of the study design (see the article "Study Design in Medical Research") and must be specified before the study starts. The study type is determined by the question to be answered and decides how useful a scientific study is and how well it can be interpreted. If the wrong study type has been selected, this cannot be rectified once the study has started.

After an earlier publication dealing with aspects of study design, the present article deals with study types in primary and secondary research. The article focuses on study types in primary research. A special article will be devoted to study types in secondary research, such as meta-analyses and reviews. This article covers the classification of individual study types. The conception, implementation, advantages, disadvantages and possibilities of using the different study types are illustrated by examples. The article is based on a selective literature research on study types in medical research, as well as the authors’ own experience.

Classification of study types

In principle, medical research is classified into primary and secondary research. While secondary research summarizes available studies in the form of reviews and meta-analyses, the actual studies are performed in primary research. Three main areas are distinguished: basic medical research, clinical research, and epidemiological research. In individual cases, it may be difficult to classify individual studies to one of these three main categories or to the subcategories. In the interests of clarity and to avoid excessive length, the authors will dispense with discussing special areas of research, such as health services research, quality assurance, or clinical epidemiology. Figure 1 gives an overview of the different study types in medical research.

Classification of different study types

This scheme is intended to classify the study types as clearly as possible. In the interests of clarity, we have excluded clinical epidemiology — a subject which borders on both clinical and epidemiological research ( 3 ). The study types in this area can be found under clinical research and epidemiology.

Basic research

Basic medical research (otherwise known as experimental research) includes animal experiments, cell studies, biochemical, genetic and physiological investigations, and studies on the properties of drugs and materials. In almost all experiments, at least one independent variable is varied and the effects on the dependent variable are investigated. The procedure and the experimental design can be precisely specified and implemented ( 1 ). For example, the population, number of groups, case numbers, treatments and dosages can be exactly specified. It is also important that confounding factors should be specifically controlled or reduced. In experiments, specific hypotheses are investigated and causal statements are made. High internal validity (= unambiguity) is achieved by setting up standardized experimental conditions, with low variability in the units of observation (for example, cells, animals or materials). External validity is a more difficult issue. Laboratory conditions cannot always be directly transferred to normal clinical practice and processes in isolated cells or in animals are not equivalent to those in man (= generalizability) ( 2 ).

Basic research also includes the development and improvement of analytical procedures—such as analytical determination of enzymes, markers or genes—, imaging procedures—such as computed tomography or magnetic resonance imaging—, and gene sequencing—such as the link between eye color and specific gene sequences. The development of biometric procedures—such as statistical test procedures, modeling and statistical evaluation strategies—also belongs here.

Clinical studies

Clinical studies include both interventional (or experimental) studies and noninterventional (or observational) studies. A clinical drug study is an interventional clinical study, defined according to §4 Paragraph 23 of the Medicines Act [Arzneimittelgesetz; AMG] as "any study performed on man with the purpose of studying or demonstrating the clinical or pharmacological effects of drugs, to establish side effects, or to investigate absorption, distribution, metabolism or elimination, with the aim of providing clear evidence of the efficacy or safety of the drug."

Interventional studies also include studies on medical devices and studies in which surgical, physical or psychotherapeutic procedures are examined. In contrast to clinical studies, §4 Paragraph 23 of the AMG describes noninterventional studies as follows: "A noninterventional study is a study in the context of which knowledge from the treatment of persons with drugs in accordance with the instructions for use specified in their registration is analyzed using epidemiological methods. The diagnosis, treatment and monitoring are not performed according to a previously specified study protocol, but exclusively according to medical practice."

The aim of an interventional clinical study is to compare treatment procedures within a patient population, which should exhibit as few as possible internal differences, apart from the treatment ( 4 , e1 ). This is to be achieved by appropriate measures, particularly by random allocation of the patients to the groups, thus avoiding bias in the result. Possible therapies include a drug, an operation, the therapeutic use of a medical device such as a stent, or physiotherapy, acupuncture, psychosocial intervention, rehabilitation measures, training or diet. Vaccine studies also count as interventional studies in Germany and are performed as clinical studies according to the AMG.

Interventional clinical studies are subject to a variety of legal and ethical requirements, including the Medicines Act and the Law on Medical Devices. Studies with medical devices must be registered by the responsible authorities, who must also approve studies with drugs. Drug studies also require a favorable ruling from the responsible ethics committee. A study must be performed in accordance with the binding rules of Good Clinical Practice (GCP) ( 5 , e2 – e4 ). For clinical studies on persons capable of giving consent, it is absolutely essential that the patient should sign a declaration of consent (informed consent) ( e2 ). A control group is included in most clinical studies. This group receives another treatment regimen and/or placebo—a therapy without substantial efficacy. The selection of the control group must not only be ethically defensible, but also be suitable for answering the most important questions in the study ( e5 ).

Clinical studies should ideally include randomization, in which the patients are allocated by chance to the therapy arms. This procedure is performed with random numbers or computer algorithms ( 6 – 8 ). Randomization ensures that the patients will be allocated to the different groups in a balanced manner and that possible confounding factors—such as risk factors, comorbidities and genetic variabilities—will be distributed by chance between the groups (structural equivalence) ( 9 , 10 ). Randomization is intended to maximize homogeneity between the groups and prevent, for example, a specific therapy being reserved for patients with a particularly favorable prognosis (such as young patients in good physical condition) ( 11 ).

Blinding is another suitable method to avoid bias. A distinction is made between single and double blinding. With single blinding, the patient is unaware which treatment he is receiving, while, with double blinding, neither the patient nor the investigator knows which treatment is planned. Blinding the patient and investigator excludes possible subjective (even subconscious) influences on the evaluation of a specific therapy (e.g. drug administration versus placebo). Thus, double blinding ensures that the patient or therapy groups are both handled and observed in the same manner. The highest possible degree of blinding should always be selected. The study statistician should also remain blinded until the details of the evaluation have finally been specified.

A well designed clinical study must also include case number planning. This ensures that the assumed therapeutic effect can be recognized as such, with a previously specified statistical probability (statistical power) ( 4 , 6 , 12 ).

It is important for the performance of a clinical trial that it should be carefully planned and that the exact clinical details and methods should be specified in the study protocol ( 13 ). It is, however, also important that the implementation of the study according to the protocol, as well as data collection, must be monitored. For a first class study, data quality must be ensured by double data entry, programming plausibility tests, and evaluation by a biometrician. International recommendations for the reporting of randomized clinical studies can be found in the CONSORT statement (Consolidated Standards of Reporting Trials, www.consort-statement.org ) ( 14 ). Many journals make this an essential condition for publication.

For all the methodological reasons mentioned above and for ethical reasons, the randomized controlled and blinded clinical trial with case number planning is accepted as the gold standard for testing the efficacy and safety of therapies or drugs ( 4 , e1 , 15 ).

In contrast, noninterventional clinical studies (NIS) are patient-related observational studies, in which patients are given an individually specified therapy. The responsible physician specifies the therapy on the basis of the medical diagnosis and the patient’s wishes. NIS include noninterventional therapeutic studies, prognostic studies, observational drug studies, secondary data analyses, case series and single case analyses ( 13 , 16 ). Similarly to clinical studies, noninterventional therapy studies include comparison between therapies; however, the treatment is exclusively according to the physician’s discretion. The evaluation is often retrospective. Prognostic studies examine the influence of prognostic factors (such as tumor stage, functional state, or body mass index) on the further course of a disease. Diagnostic studies are another class of observational studies, in which either the quality of a diagnostic method is compared to an established method (ideally a gold standard), or an investigator is compared with one or several other investigators (inter-rater comparison) or with himself at different time points (intra-rater comparison) ( e1 ). If an event is very rare (such as a rare disease or an individual course of treatment), a single-case study, or a case series, are possibilities. A case series is a study on a larger patient group with a specific disease. For example, after the discovery of the AIDS virus, the Center for Disease Control (CDC) in the USA collected a case series of 1000 patients, in order to study frequent complications of this infection. The lack of a control group is a disadvantage of case series. For this reason, case series are primarily used for descriptive purposes ( 3 ).

Epidemiological studies

The main point of interest in epidemiological studies is to investigate the distribution and historical changes in the frequency of diseases and the causes for these. Analogously to clinical studies, a distinction is made between experimental and observational epidemiological studies ( 16 , 17 ).

Interventional studies are experimental in character and are further subdivided into field studies (sample from an area, such as a large region or a country) and group studies (sample from a specific group, such as a specific social or ethnic group). One example was the investigation of the iodine supplementation of cooking salt to prevent cretinism in a region with iodine deficiency. On the other hand, many interventions are unsuitable for randomized intervention studies, for ethical, social or political reasons, as the exposure may be harmful to the subjects ( 17 ).

Observational epidemiological studies can be further subdivided into cohort studies (follow-up studies), case control studies, cross-sectional studies (prevalence studies), and ecological studies (correlation studies or studies with aggregated data).

In contrast, studies with only descriptive evaluation are restricted to a simple depiction of the frequency (incidence and prevalence) and distribution of a disease within a population. The objective of the description may also be the regular recording of information (monitoring, surveillance). Registry data are also suited for the description of prevalence and incidence; for example, they are used for national health reports in Germany.

In the simplest case, cohort studies involve the observation of two healthy groups of subjects over time. One group is exposed to a specific substance (for example, workers in a chemical factory) and the other is not exposed. It is recorded prospectively (into the future) how often a specific disease (such as lung cancer) occurs in the two groups ( figure 2a ). The incidence for the occurrence of the disease can be determined for both groups. Moreover, the relative risk (quotient of the incidence rates) is a very important statistical parameter which can be calculated in cohort studies. For rare types of exposure, the general population can be used as controls ( e6 ). All evaluations naturally consider the age and gender distributions in the corresponding cohorts. The objective of cohort studies is to record detailed information on the exposure and on confounding factors, such as the duration of employment, the maximum and the cumulated exposure. One well known cohort study is the British Doctors Study, which prospectively examined the effect of smoking on mortality among British doctors over a period of decades ( e7 ). Cohort studies are well suited for detecting causal connections between exposure and the development of disease. On the other hand, cohort studies often demand a great deal of time, organization, and money. So-called historical cohort studies represent a special case. In this case, all data on exposure and effect (illness) are already available at the start of the study and are analyzed retrospectively. For example, studies of this sort are used to investigate occupational forms of cancer. They are usually cheaper ( 16 ).

In case control studies, cases are compared with controls. Cases are persons who fall ill from the disease in question. Controls are persons who are not ill, but are otherwise comparable to the cases. A retrospective analysis is performed to establish to what extent persons in the case and control groups were exposed ( figure 2b ). Possible exposure factors include smoking, nutrition and pollutant load. Care should be taken that the intensity and duration of the exposure is analyzed as carefully and in as detailed a manner as possible. If it is observed that ill people are more often exposed than healthy people, it may be concluded that there is a link between the illness and the risk factor. In case control studies, the most important statistical parameter is the odds ratio. Case control studies usually require less time and fewer resources than cohort studies ( 16 ). The disadvantage of case control studies is that the incidence rate (rate of new cases) cannot be calculated. There is also a great risk of bias from the selection of the study population ("selection bias") and from faulty recall ("recall bias") (see too the article "Avoiding Bias in Observational Studies"). Table 1 presents an overview of possible types of epidemiological study ( e8 ). Table 2 summarizes the advantages and disadvantages of observational studies ( 16 ).

Selecting the correct study type is an important aspect of study design (see "Study Design in Medical Research" in volume 11/2009). However, the scientific questions can only be correctly answered if the study is planned and performed at a qualitatively high level ( e9 ). It is very important to consider or even eliminate possible interfering factors (or confounders), as otherwise the result cannot be adequately interpreted. Confounders are characteristics which influence the target parameters. Although this influence is not of primary interest, it can interfere with the connection between the target parameter and the factors that are of interest. The influence of confounders can be minimized or eliminated by standardizing the procedure, stratification ( 18 ), or adjustment ( 19 ).

The decision as to which study type is suitable to answer a specific primary research question must be based not only on scientific considerations, but also on issues related to resources (personnel and finances), hospital capacity, and practicability. Many epidemiological studies can only be implemented if there is access to registry data. The demands for planning, implementation, and statistical evaluation for observational studies should be just as high for observational studies as for experimental studies. There are particularly strict requirements, with legally based regulations (such as the Medicines Act and Good Clinical Practice), for the planning, implementation, and evaluation of clinical studies. A study protocol must be prepared for both interventional and noninterventional studies ( 6 , 13 ). The study protocol must contain information on the conditions, question to be answered (objective), the methods of measurement, the implementation, organization, study population, data management, case number planning, the biometric evaluation, and the clinical relevance of the question to be answered ( 13 ).

Important and justified ethical considerations may restrict studies with optimal scientific and statistical features. A randomized intervention study under strictly controlled conditions of the effect of exposure to harmful factors (such as smoking, radiation, or a fatty diet) is not possible and not permissible for ethical reasons. Observational studies are a possible alternative to interventional studies, even though observational studies are less reliable and less easy to control ( 17 ).

A medical study should always be published in a peer reviewed journal. Depending on the study type, there are recommendations and checklists for presenting the results. For example, these may include a description of the population, the procedure for missing values and confounders, and information on statistical parameters. Recommendations and guidelines are available for clinical studies ( 14 , 20 , e10 , e11 ), for diagnostic studies ( 21 , 22 , e12 ), and for epidemiological studies ( 23 , e13 ). Since 2004, the WHO has demanded that studies should be registered in a public registry, such as www.controlled-trials.com or www.clinicaltrials.gov . This demand is supported by the International Committee of Medical Journal Editors (ICMJE) ( 24 ), which specifies that the registration of the study before inclusion of the first subject is an essential condition for the publication of the study results ( e14 ).

When specifying the study type and study design for medical studies, it is essential to collaborate with an experienced biometrician. The quality and reliability of the study can be decisively improved if all important details are planned together ( 12 , 25 ).

What are the types of clinical trials?

There are different types of clinical trials.

Why do researchers do different kinds of clinical studies?

  • Prevention trials look for better ways to prevent a disease in people who have never had the disease or to prevent the disease from returning. Approaches may include medicines, vaccines, or lifestyle changes.
  • Screening trials test new ways for detecting diseases or health conditions.

What are the phases of clinical trials?

Clinical trials are conducted in a series of steps called “phases.” Each phase has a different purpose and helps researchers answer different questions.

  • Phase I trials : Researchers test a drug or treatment in a small group of people (20–80) for the first time. The purpose is to study the drug or treatment to learn about safety and identify side effects.
  • Phase II trials : The new drug or treatment is given to a larger group of people (100–300) to determine its effectiveness and to further study its safety.
  • Phase III trials : The new drug or treatment is given to large groups of people (1,000–3,000) to confirm its effectiveness, monitor side effects, compare it with standard or similar treatments, and collect information that will allow the new drug or treatment to be used safely.
  • Phase IV trials : After a drug is approved by the FDA and made available to the public, researchers track its safety in the general population, seeking more information about a drug or treatment’s benefits, and optimal use.

What do the terms placebo, randomization, and blinded mean in clinical trials?

In clinical trials that compare a new product or therapy with another that already exists, researchers try to determine if the new one is as good, or better than, the existing one. In some studies, you may be assigned to receive a placebo (an inactive product that resembles the test product, but without its treatment value).

Comparing a new product with a placebo can be the fastest and most reliable way to show the new product’s effectiveness. However, placebos are not used if you would be put at risk — particularly in the study of treatments for serious illnesses — by not having effective therapy. You will be told if placebos are used in the study before entering a trial.

Randomization is the process by which treatments are assigned to participants by chance rather than by choice. This is done to avoid any bias in assigning volunteers to get one treatment or another. The effects of each treatment are compared at specific points during a trial. If one treatment is found superior, the trial is stopped so that the most volunteers receive the more beneficial treatment.  This video helps explain randomization for all clinical trials .

" Blinded " (or " masked ") studies are designed to prevent members of the research team and study participants from influencing the results. Blinding allows the collection of scientifically accurate data. In single-blind (" single-masked ") studies, you are not told what is being given, but the research team knows. In a double-blind study, neither you nor the research team are told what you are given; only the pharmacist knows. Members of the research team are not told which participants are receiving which treatment, in order to reduce bias. If medically necessary, however, it is always possible to find out which treatment you are receiving.

Who takes part in clinical trials?

Many different types of people take part in clinical trials. Some are healthy, while others may have illnesses. Research procedures with healthy volunteers are designed to develop new knowledge, not to provide direct benefit to those taking part. Healthy volunteers have always played an important role in research.

Healthy volunteers are needed for several reasons. When developing a new technique, such as a blood test or imaging device, healthy volunteers help define the limits of "normal." These volunteers are the baseline against which patient groups are compared and are often matched to patients on factors such as age, gender, or family relationship. They receive the same tests, procedures, or drugs the patient group receives. Researchers learn about the disease process by comparing the patient group to the healthy volunteers.

Factors like how much of your time is needed, discomfort you may feel, or risk involved depends on the trial. While some require minimal amounts of time and effort, other studies may require a major commitment of your time and effort, and may involve some discomfort. The research procedure(s) may also carry some risk. The informed consent process for healthy volunteers includes a detailed discussion of the study's procedures and tests and their risks.

A patient volunteer has a known health problem and takes part in research to better understand, diagnose, or treat that disease or condition. Research with a patient volunteer helps develop new knowledge. Depending on the stage of knowledge about the disease or condition, these procedures may or may not benefit the study participants.

Patients may volunteer for studies similar to those in which healthy volunteers take part. These studies involve drugs, devices, or treatments designed to prevent,or treat disease. Although these studies may provide direct benefit to patient volunteers, the main aim is to prove, by scientific means, the effects and limitations of the experimental treatment. Therefore, some patient groups may serve as a baseline for comparison by not taking the test drug, or by receiving test doses of the drug large enough only to show that it is present, but not at a level that can treat the condition.

Researchers follow clinical trials guidelines when deciding who can participate, in a study. These guidelines are called Inclusion/Exclusion Criteria . Factors that allow you to take part in a clinical trial are called "inclusion criteria." Those that exclude or prevent participation are "exclusion criteria." These criteria are based on factors such as age, gender, the type and stage of a disease, treatment history, and other medical conditions. Before joining a clinical trial, you must provide information that allows the research team to determine whether or not you can take part in the study safely. Some research studies seek participants with illnesses or conditions to be studied in the clinical trial, while others need healthy volunteers. Inclusion and exclusion criteria are not used to reject people personally. Instead, the criteria are used to identify appropriate participants and keep them safe, and to help ensure that researchers can find new information they need.

In rare cases, participants have been seriously injured or have died of complications resulting from their participation in trials of experimental treatments. What happens after a clinical trial is completed?

After a clinical trial is completed, the researchers carefully examine information collected during the study before making decisions about the meaning of the findings and about the need for further testing. Researchers explore generative AI benefits and shortfalls in medical education

AI is a rapidly growing technology that is now influencing the world of medical education. For educators, this emerging technology is showing its potential in bringing real value to classrooms. That said, the use of AI has led to the question about whether these intelligent systems can play a significant role in medical institutions.

Academia, including medical education , has become divided in this new arena due to ethical concerns arising over misuse, cheating, and privacy when using AI.

To better understand the current evidence and best practices behind how to implement generative AI in teaching and medical education, one study—led by third-year UNC School of Medicine student Joshua Hale—is aiming to provide clarity for educators and students about best practices for implementation of these tools, as well as identify gaps in the literature to provide future investigation into why AI should be included in the classroom and curriculum.

"We found some interesting ideas about how generative AI can augment professors and students," said Hale. "Specifically, many articles suggested that generative AI's natural language processing ability will make them good candidates for future implementation as virtual patients, enabling students to practice independently and receive personalized feedback."

The study, published in the Journal of Medical Education and Curricular Development , reviewed 521 articles that were released before June 30, 2023, on how generative AI is and could be used and implemented in undergraduate medical education (UME).

A total of 41 articles were extracted and showed how the majority of them were opinion pieces, case reports, letters to the editor, editorials, and commentaries about the use of generative AI while seven articles used qualitative and/or quantitative methods.

The literature was also divided into five categories acknowledging the role AI can implement in UME as well as its challenges: nonclinical learning assistant, content developer, virtual patient interaction, clinical decision-making tutor, and medical writing. Results also showed that generative AI tools' greatest potential is for use as a virtual patient and clinical decision-making tutor.

Identifying whether these areas will yield the most significant benefits is a question researchers aimed to address as they searched for the integration of AI tools into medical education. The study further explored how these five categories were proposed to enhance undergraduate medical education based on current literature.

"Nonclinical learning assistant" was the first in which articles suggested the use of AI as a reference for students to search for and find medical information. Virtual assistant ChatGPT 3.5 showed how it could answer clinical free-response questions above a passing threshold 43% of the time. Articles suggested that AI could be used as a learning assistant or personalized tutor for nonclinical content.

"Content developer" appeared in articles discussing the ability of ChatGPT to write multiple-choice questions and provide explanations. Researchers found that AI could write multiple-choice questions at a similar level of proficiency to that of a human in significantly less time.

For "curriculum," it was found that AI could develop cases and simulations and suggest the implementation of guidelines and detectors to prevent misuse by students. Four articles provided insight into how AI models could create a type of virtual patient interaction and tutoring. It will be able to display how ChatGPT could act as a virtual patient and provide feedback to students on their clinical decision-making, as well as a clinical reasoning tutor.

"Generative AI could simulate virtual patients that respond to student queries with specific diagnoses or learning objectives in mind," said Hale. "While current virtual patients exist, generative AI has the capability to interpret text and produce unique responses based on student interactions.

"For example, students could conduct interviews with AI virtual patients on their computers, receive follow-up questions, and obtain automated feedback on their performance. This technology expands the potential educational tools available to educators," he said.

Lastly, medical writing was frequently cited as an area of use and an area of concern. AI displayed creative writing assistance, research manuscripts, and grant writing as well as support in writing medical notes. To help with the concern of misusing the tool in this category, researchers recommended the implementation of surveillance systems for generative AI use and tailoring assignments towards its limitations, so students are forced to address its shortfalls.

"Generative AI-generated content can evade automated detection, posing challenges to academic integrity," said Hale. "Tools like ChatGPT are valuable for generating search strategies and brainstorming. Schools are increasingly acknowledging these capabilities and educating students on appropriate usage and citation of generative AI."

The study suggests that ideas put forth in these articles can provide a starting point for educators looking to innovate in the classroom and prepare students for a medical field that increasingly embraces AI tools. Advantages of these tools can lead to personalized insight into cognitive or practical mistakes that the student can possibly make and provide immediate feedback.

By incorporating AI and its new capabilities, educators can spend more time focusing on the next generation of medical professionals and innovation. However, Hale emphasized the need for more research.

"Further research is essential to demonstrate the specific benefits of integrating AI tools into classroom learning," said Hale. "Nevertheless, these tools are already proving to be valuable adjuncts for interested users.

"My classmates frequently leverage ChatGPT as a study partner to develop cognitive aids, study templates, and more. In a research setting, it can serve as a resource for generating ideas for literature searches, learning coding or data analysis, and much more."

As the mountains and trees of California’s Napa Valley drift past the car window, 6-year-old David Kastner is deep in conversation with his father. The conversation is a familiar one, shifting naturally from gravity to electromagnetism. For as long as he can remember, scientific curiosity has been a key part of his conversations on these drives.

“I remember being fascinated by how complex the universe is and how little people know about it,” recalls Kastner, now a fourth-year PhD student in biological engineering. “I always wanted to uncover new truths about the universe.”

Nearly two decades later, Kastner is now at MIT studying a challenging subset of proteins known as metalloenzymes, in the lab of Heather Kulik, a professor of chemical engineering, and Forest White, a professor of biological engineering. With the same curiosity that sparked those on-the-road discussions with his father, Kastner is motivated by a desire to harness the chemical and medical potential of enzymes through computational and mechanistic approaches.

Kastner’s research aims to uncover the fundamental blueprints of reactivity for enzymes using state-of-the-art computational methods. However, his approach to research involves not just physics, chemistry, and biology, but also art, which has been an integral part of his life since childhood. Kastner produces beautiful 3D illustrations of molecular systems that help make his research more accessible to a wider audience.

“Seeing the science in a way that looks so real that you feel like you can touch it can be more impactful than a bar plot or a histogram,” he says. “If scientists were more invested in showing their work in engaging and interesting ways, then we would have more people involved in science.”

Form and function in equal measure

Kastner’s research has spanned quantum chemistry calculations, protein engineering, bioinformatics, synthetic organic chemistry, and mammalian tissue models. He earned his bachelor’s degree in biophysics at Brigham Young University, and once he began his PhD program at MIT, he decided to zero in on metalloenzymes.

Among metalloenzymes, Kastner has chosen to focus on high-valent metalloenzymes, which contain a highly reactive metal atom that has lost many of its electrons and eagerly reacts to regain them. His personal favorites are non-heme iron enzymes, due to their vast repertoire of chemical reactions, direct applicability to human health, and the tunability of their active sites for engineering novel reactivities.

Giving old enzymes new reactivities isn’t easy, however. His first published paper, authored alongside former members of the Kulik Research Group, showed why.

Kastner’s research explores the mechanistic differences between non-heme iron halogenases and hydroxylases, two classes of high-valent enzymes that activate normally unreactive C–H bonds. By investigating trends across structural databases and molecular dynamics simulations, he identified key interactions that result in subtle differences in the substrate positioning angle, influencing reactivity. Kastner’s computational findings suggest new ways of converting between halogenases and hydroxylases.

While an intuition of an enzyme’s structure can go a long way, sometimes you need to move beyond structure. “As soon as you add a metal into the core of an enzyme, it becomes much more challenging to model,” he says. “It requires unique and cutting-edge tools in order to understand reactivity. That’s why we need quantum chemistry calculations so much in our research.”

Trying to unlock the secrets of nature’s most efficient catalysts requires observations at the sharpest level possible. A given enzyme’s structure and reactivity is determined by the interactions between the electrons it contains, hence the reliance on quantum computing methods.

The importance of viewing the entire enzyme from a quantum mechanical lens came to the forefront of Kastner’s research in his most recent publication. Kastner and his collaborators discovered that the reactivity of a class of miniature artificial metalloenzymes was controlled by changes in dynamic charge distributions, which can be thought of as a way of seeing how electrons and charges fluctuate throughout an enzyme’s structure.

“If you’re interested in how life functions, then it only makes sense to look at enzymes and proteins,” he says. “Enzymes are the machinery that evolution came up with to harness physics and chemistry.”

“I’ve always been interested in that question,” he continues. “How do you get from these purely mathematical underlying physical laws to living, breathing organisms with feelings?”

The art of science

In addition to research, Kastner can be found using 3D graphics programs like Blender and VMD to visualize macromolecular systems and their interactions. His work can be seen on the covers of scientific journals published by  Nature and the  American Chemical Society , but his initial forays into art were far simpler.

“I would draw everything,” he says. “It was the game I would play. I would draw; I would ask my parents to draw for me; I would ask people I would meet, ‘Can you draw this for me?’”

His mother made hyperrealistic art inspired by nature and was the biggest artistic influence on him early on. Kastner described a photorealistic lynx his mother drew with a scratch board hanging at his grandparents’ home that he found particularly inspiring as a child.

He took traditional art quite seriously in high school. He worked with charcoal and oils, winning multiple competitions, but he wasn’t sure how he might apply these skills to his academic interests.

“At that time, I hadn’t realized how to reconcile art and my love of science,” he says. “They still felt so different and no one I talked to tried to combine them at all.”

If he had come of age in late-15th-century Italy, however, that might not have been the case. The Renaissance was defined by figures who didn’t see boundaries between various disciplines, and perhaps none are more enduring than Kastner’s favorite scientist of all time: Leonardo da Vinci.

“It’s pretty incredible that the man who is universally credited as being the grandfather of modern anatomy and physiology is also the same man who painted the ‘Mona Lisa,’” he says. “I feel like the world would be a better place if we had more people like da Vinci who could reconcile the sciences and art.”

In fact, he thinks the erosion of trust in scientists could be eased if that were the case. Peer-reviewed papers are dense and technical because they need to describe complex experiments in a way that makes their results reproducible, but that means the average person probably won’t understand it. That’s where art can help bridge the gap.

“If we communicate our science in ways that connect to ordinary people, I think it will automatically get rid of some of that distrust,” he says. “We need to keep writing papers the way we do; there’s no way around that. However, scientific art can help make this information more accessible. By converting esoteric data into familiar and relatable visuals, researchers can extend an invitation to people of all ages and backgrounds to interact with their science through the universally shared language of art.”

Summer Internship Program & Clinical Electives Program

The  NIH Clinical Center Summer Internship Program  offers currently enrolled students the opportunity to work with NIH researchers and health professionals during their summer months.

The NIH Clinical Electives Program offers year-round clinical opportunities for senior medical students to gain clinical research exposure through rotations with services that are responsible for the clinical care of patients enrolled in protocols and clinical trials.

Graduate Medical Education Research Committee

The Graduate Medical Education (GME) Research Committee at the University of Arizona College of Medicine – Tucson is dedicated to fostering a vibrant research environment for medical trainees at all levels. Research 101

There is a lot of clinical research-related education available online. The following primer was written by Dr. Spencer Moore, ophthalmology resident and co-director of education for the GME Research Committee.

Study Types

Retrospective: the entire event being studied started, took place, and was completed in the past

Cross-sectional: only a single point in time

Prospective: recruit subjects in present and follow longitudinally over time

Single-armed: only 1 group (e.g., people who smoke)

Double-armed: multiple groups studied (e.g., control/placebo arm and intervention/exposure arm)

Intervention

None/observational: subjects are studied for the association between an independent variable (e.g., smoking status) and dependent variable/outcome (e.g., development of lung cancer)

Interventional: subjects are grouped according to some treatment or intervention received

Single-masked: subjects are masked to the status of treatment vs. placebo

Double-masked: subjects are masked to the status of treatment vs. placebo AND investigator is masked to subjects’ group assignment

Randomization

Only possible for prospective interventional cohort studies; avoids selection and allocation biases inherent to nonrandomized study design. Allows for evidence of causality between independent (e.g., treatment/intervention) and dependent (e.g., outcome) variables.

Elements of Research Proposal

A well-referenced and concise written research proposal is essential. Depending on where you are sending it (UA RIA, UA IRB, SAVAHCS, Banner, private funding agencies, etc.), there may be differences in the format. In general, the format will include the following elements:

Abstract:  May or may not be included depending on format. It should be 1 paragraph and about 250-300 words long. It would include a brief few sentences of background and introduction. This should lead into the knowledge gap in the field and set up the reader for the hypothesis to be tested. Next, the study design and aims should be stated as briefly and concisely as possible. Next, the hypothesis should be stated, followed by the importance or significance of potential results in the field. The abstract generally does not contain references.

Background & Introduction : Should be about 1 page or less of single-spaced background and introduction to the topic being studied, usually for a general medical or scientific audience. This section should be well referenced. Introduce the condition being studied and some of its public health implications. Then describe what is known in the field about your specific topic. This should progress logically toward the question your study seeks to answer. You may conclude this section with something to the effect of, “In the present study, we plan to use [insert study design here] to answer the question of [insert major research question or knowledge gap here].”

Specific Aims: Depending on the format of the study, this may be a single aim with several sub-aims, or multiple specific aims, each with their own sub-aim. Describe the format of the study, the patient population to be studied, means of recruitment, data points to be collected, type of statistical analysis and comparisons to be performed. Include a testable hypothesis related to the aim described. Each aim should be about 1 paragraph.

Protocol : This will include a detailed account of the experimental protocol. It may be several pages long. It will vary according to your specific project. Organize it according to each specific aim. It will include a detailed description of the patient population being studied, how those patients will be selected or recruited, the source of data to be collected, the data points that you will collect and how those data will be analyzed. You should also include a selection relating to human subjects protection that describes how clinical data will be safeguarded, how human subject identities will be protected and deidentified, how data will be stored, etc. If subjects are being actively recruited and consented for a study there will be an additional section describing the informed consent process and how subjects will be recruited. Include a primary outcome measure in your data to be collected and a description of how this will be calculated. For example a primary outcome measure in a prospective observational cohort study of smokers and nonsmokers might be the development of lung cancer within 10 years, and the statistical analysis to be performed might be relative risk of developing lung cancer within 10 years of enrollment. If there is any prospective component to your research component, where patients are brought in for testing or treatment, then you will need to identify a funding source — your research mentor can assist you.

Power analysis: For a prospective study, it is important to have some statistical basis to calculate the number of subjects you plan to recruit. For a retrospective study, this can also be helpful to determine the number of charts needed to review. There are various online sample size calculators as part of your power analysis.

It is important to have a reasonable grasp of statistics in your research protocol. Understand exactly what clinical data will be collected so that you can propose the appropriate statistical analysis.

Some common statistical tests:

t test: to compare means of 2 groups with continuous numerical data. Generally it would be a 2-tailed, unpaired t-test. For example, you could compare the mean age of 2 groups.

Chi-square or Fisher exact test: compares the proportions of 2 subjects in 2 groups. For example, you could compare the percentage of subjects in 2 groups who develop lung cancer.

ANOVA: to compare group means of a dependent variable with multiple different levels of the independent variable, for example systolic blood pressure levels with different dosages of blood pressure medications.

Kaplan Meier: comparing survival, or latency to meet some primary outcome measure over time. Can calculate a Kaplan Meier or log-rank statistic to compare survival in 2 different groups.

You will probably need 2 separate and parallel research applications approved before starting your project if using the UA research infrastructure and IRB. Your department likely has research coordinators who can help you and determine any documents you will need. This will probably include approvals from the Research Intake Application (RIA) and an Institutional Review Board (IRB).

UA REDCap is a great secure way to store clinical data. If you will be collecting lots of separate data points, REDCap is an efficient way to store, organize and sort data that can easily be anonymized and exported.

Step by Step: Conducting Research

Conducting research as a trainee can be daunting. Consider the following steps to get you started with conducting a human research project, and please also discuss with your mentor. For an explanation of the full IRB process, review the “Getting Started” webpage at RII .

Download the appropriate forms from the IRB website :

  • IRB protocol
  • consent forms
  • applicable appendices
  • attestations from your adviser, scientific reviewer, and department head

Begin eIRB submission

Begin your submission at the eIRB website (NetID required). This how-to video may help you to complete this step.

If applicable, submit to Banner

If Banner staff, patients, medical records or specimens will be used, submit your study through the Research Administration Portal (RAP) (NetID required).

Add Banner approval to eIRB

Once RAP is approved, add the approval email to your study in eIRB and press “Submit” for your IRB to be formally submitted. An IRB coordinator will be assigned to pre-review your study prior to final IRB chair review/approval. Anticipate receiving comments and questions for further revision during this phase. Once the study is approved, you will receive an eIRB system-generated email. Make sure to review your approval letter to understand reporting requirements moving forward. You can then begin your research.

Work with statistician

Once the data from the project is available for analysis, work with a statistician to help with the pre-defined statistical analysis plan, and interpret the results to draw meaningful conclusions related to the research question.

Share your results

Consider presenting your research at local and or national meetings, as you are collecting your data with the ultimate goal of a peer-reviewed manuscript publication.
