SUPPORTING DOCUMENTS

Supporting documents for appeals, reviews and extension requests provide curtin with evidence you are experiencing extenuating circumstances for a period of time.  , document guidelines .

The supporting documents that you submit with your application will need to cover the date of the assessment . For example, a medical certificate needs to cover the date the assignment was due, or the date of your examination. Your application may be unsuccessful if the date you are unfit is reflected after the assessment or exam is due.  Things to remember when providing scanned supporting documents:

  • Documents must be photographed from the original document in colour.
  • Documents must be placed on a flat and plain background when photographed.
  • Information in the photograph must be clear and legible. Check the document (e.g. to zoom in) and confirm fine details are clear before submitting the document. What looks good on your phone may be blurred on a desktop computer. 
  • Information in the photograph must not be obstructed, for example, by your fingers or a shadow.
  • Your name and dates on the document must be clear and legible. 

What type of document can I use?

Curtin considers the following to be valid types of supporting documentation as per the Assessment Extension Criteria found in the Assessment and Student Progression Manual :  

Student injury, illness or medical condition

Some examples of these documents are:

Signed statement (e.g., medical certificate) on relevant official letterhead from a Medical Practitioner or other approved health professional registered by the relevant National Medical Board ( www.medicalboard.gov.au ). Medical statements from online services will not be accepted unless face to face consultation occurs or a previous practitioner-patient relationship exists in accordance with the AMA Medical Certificate Guidelines, revised 2016.  Medical statements from pharmacists are not acceptable.

Signed statement on relevant official letterhead from the University’s Psychological and Counselling Services or AccessAbility Services , outlining how circumstances have impacted the student’s ability to submit the assessment item on the due date.

Triage appointments are held between 10:00am and 1:00pm, Monday to Friday. We recommend that you call prior to 9:00am to ensure a same-day appointment.

Family issues

Some examples of this would include family injury, illness, or bereavement.

  • Death Certificate or funeral notice.  
  • Police report or statement.  
  • Any form of supporting documentation accepted under the Student injury, illness or medical condition category listed above.  

We suggest that you explain your family dynamic and why this event may have assigned you more responsibilities, or the significance of your relationship. For example, if you took on the role of being the caretaker for younger siblings, what that involved. 

Participation in Defence Force or Emergency Services

  • Emergency Services: Signed statement of support on relevant official letterhead detailing the mandatory service requirements or participation in a recognised Emergency Services callout.  
  • Defence Force: Signed statement on relevant official letterhead detailing the student is rendering Defence Service

Unavoidable and unexpected work commitments not opted into on a voluntary basis

  • Signed statement on company letterhead from the employer supporting claims of unavoidable and unexpected work commitments, confirming that these were not opted into on a voluntary basis.   
  • For self-employed students, explicit detail explaining why work commitments are both unavoidable and unexpected will be provided.

Elite Athlete sporting activity participation

  • Signed statement of support from the University’s Elite Athlete Coordinator outlining how circumstances have impacting the student’s ability to submit the assessment item on the due date.

Obligatory Religious Requirements

  • Signed statement of support from the University’s Multifaith Services outlining how circumstances have impacted the student’s ability to submit the assessment item on the due date. 

Further reasons including social and welfare

  • Signed statement of support from the University’s Student Wellbeing Advisor outlining how circumstances have impacted the student’s ability to submit the assessment item on the due date.

  • Other reasons will be considered on a case by case basis. We encourage you to explain your situation in detail for Curtin staff to understand why it is not possible for you to provide supporting documents for your circumstances. Keep in mind that a Statutory Declaration should cover all “other” circumstances.

  • Ensure you are using a supported browser such as Google Chrome, Safari or Mozilla Firefox (Internet Explorer or MS Edge should not be used).
  • If you are using a MAC, ensure no special characters (e.g.' /' or ':' ) are included in the filename of any attachment. Keep the name of attachment simple, e.g. johnsmith.pdf
  • Ensure you have disabled The Grammarly Browser extension on the site.
  • If the list of assessments for a unit shows, but the assessment wanted is not listed, please check the Unit Outline. The application will need to be submitted under one of the listed assessments. It may be named differently or be part of a larger assessment. Once an assessment is chosen, you can enter in more information about the assessment in the additional assessment information  box.
